About finasteride

  • Finasteride 1mg is a generic prescription-only medication taken daily to treat male pattern hair loss in men. It is also available under the brand name Propecia.
  • Finasteride reduces the amount of testosterone converted to DHT (dihydrotestosterone) which results in hair regrowth and renewed hair thickness in most men.
  • At 5 years of taking finasteride 1mg men, have a 90% chance of having more or the same amount of hair.
  • Read the manufacturer's patient leaflet for a full list of side effects, cautions, and interactions with other drugs.
  • Finasteride can also be used to treat benign prostate enlargement (BPE/BPH), but with a different dose of 5mg daily.

Generic finasteride 1mg and Propecia

The UK patent for Propecia expired in 2014. 'Generic Propecia' or more accurately 'generic finasteride' became available from other pharmaceutical companies. A generic medicine is a medicine which is the same as a branded original, but is prescribed and supplied by its medical name, rather than its brand name.

Propecia is a brand name. The active ingredient of Propecia tablets is finasteride 1mg. Propecia tablets and finasteride 1mg tablets are medically the same. Dr Fox may supply any UK licensed generic version (Accord, Amarox, Bristol, Mylan, Milpharm/Aurobindo, Relonchem).

How long does it take to work?

Clinical studies show improved hair growth usually noticeable after 4-6 months of taking finasteride 1mg daily. Maximum benefit is seen at 1-2 years. At 2 years 66% of men had improved hair growth counts compared to 7% of those taking no drug.

The studies were conducted on men aged 18 to 41 years of age with mild to moderate hair loss, but not complete hair loss.

How does finasteride work?

Hair grows in hair follicles normally for about three years. The hair then falls out and a new hair grows from the follicle. For men affected by male pattern hair loss, as they age, hair follicles gradually become smaller. New hair becomes thinner and falls out more quickly, eventually not growing enough to break the surface of the scalp (balding).

Male hormones cause these changes. Hair follicles convert the male hormone testosterone into another hormone called dihydrotestosterone (DHT). Hair follicles become more sensitive to dihydrotestosterone and this causes the hair follicles to shrink.

Finasteride 1mg blocks testosterone conversion to DHT which results in hair regrowth and improved hair thickness in most men.

Who benefits

Men at any stage of male pattern hair loss are likely to benefit. Finasteride 1mg is most effective for hair loss over the top of the scalp (vertex). It has NOT been proven to reverse receding frontal hairline, hair loss which is ONLY in the temple regions, nor restore hair to entirely bald areas.

Cautions

  • If hair loss has been rapid and recent, or where there are symptoms of serious illness, such as unexplained weight loss, fevers, persistent diarrhoea, or excessive fatigue, you should see your GP.
  • If hair loss is patchy or irregular, or comes out in clumps, or there is scalp inflammation, the scalp should be examined by a GP or dermatologist.
  • Blood tests carried out to detect prostate cancer (PSA) can give misleading results when finasteride 1mg is being taken. Tell your doctor that you are taking finasteride, if you are having a PSA test.
  • If treatment is stopped, the beneficial effects begin to reverse by six months and return to baseline by 9 to 12 months.
  • Pregnant women should not come into contact with finasteride. It may affect the baby's sex organ development. For this reason tablets are film-coated and women should not handle crushed or broken tablets.

What are the possible side effects of finasteride?

Finasteride 1mg is usually well tolerated by most men and side effects are uncommon. Men taking finasteride are at increased risk for erectile dysfunction, decreased libido, or other ejaculation disorder with up to 1 in 27 reporting changes in the first year. This is not generally severe enough to prevent continued use (1% mention it as a reason for stopping).

There are some case reports of diminished libido or erectile dysfunction persisting after stopping the drug and the patient leaflet has been updated to reflect this.

In 2017 the NHS reviewed new research and concluded that If sexual problems do occur, the drug can be stopped and the problem will resolve in nearly all cases. An editorial in the British Medical Journal in 2019 supported this position, but suggested a need for further research.

Other side effects can include breast tenderness, gynaecomastia (enlarged breast tissue in men), and anxiety or depression.

Important: if you develop any lumps or experience pain in your breast area, or discharge from your nipples, please consult your GP. There have been rare reports of breast cancer in men who have taken finasteride.

Some men have reported episodes of depressive illness in association with the use of finasteride 1mg for male pattern hair loss. There are some reports of men having suicidal thoughts. If low mood or depression develops stop treatment immediately and consult your GP straight away.

Regaine Extra Strength (minoxidil 5%) foam or lotion applied to the scalp twice daily is the only other proven effective hair loss treatment. Regaine and finasteride 1mg can be used at the same time.

Why should men see a doctor before starting treatment?

Finasteride 1mg is a treatment for male pattern hair loss. This is the most common, but not the only type of hair loss. A doctor can confirm the cause and where necessary and undertake tests to see if other factors are causing hair loss and whether other treatments are needed.

Is finasteride used to treat anything else apart from male pattern hair loss?

Yes. A high dose is sometimes prescribed to treat prostate enlargement. For this indication 5mg tablets are used, whereas for hair loss the tablets are 1mg.

Will finasteride work better if I take a higher dose?

Hair loss is treated with one 1mg tablet of finasteride daily. Taking more simply increases the side effects without improving the results. Studies have confirmed this.

Can I take less than finasteride 1mg daily or take it less often, to reduce the risk of side effects?

Many websites suggest either taking finasteride 1mg for only 3 days a week or splitting the tablets and taking either 0.25mg or 0.5mg daily, to reduce the risk of side effects from taking finasteride to treat male pattern hair loss.

Finasteride works by suppressing the conversion of testosterone to DHT (dihydrotestosterone) which causes shrinkage of hair follicles and hair loss. Research has shown that 1mg finasteride daily maintains this suppression in the majority of men. A lower dose, as in splitting the tablets, may sometimes be sufficient to suppress the effect of DHT on follicles. However, a lower than 1mg dose will not be enough for most men.

The tablets are not easy to split and may crumble, as they are film coated. It is extremely important that broken or crushed pills are not inadvertently touched by pregnant women or women who may become pregnant, as the active ingredient can be damaging to an unborn child. The film coating is an extra protection, therefore splitting finasteride pills is not recommended.

Does finasteride work in women?

No. Thinning of hair and hair loss in women is different from that in men, and it does not respond to finasteride. Finasteride has potentially serious side effects, particularly in women of childbearing age, and should not be taken by women.

Does finasteride work in older men?

Studies on the effectiveness of finasteride 1mg have been undertaken in men up to the age of 41 years. Many older men take finasteride 1mg for hair loss. Theoretically the drug should be effective in older men, but the studies to confirm this have not been carried out or are not published.

Does finasteride work in all men?

Not all men respond to treatment. If there is no improvement in 6 months, treatment should be stopped. Details of efficacy can be found in study data from medical trials.

Does finasteride reduce sex drive and fertility?

There is much controversy online about this subject. The manufacturer's patient information leaflet provides details and the NHS reviewed new research regarding these concerns in 2017 and concluded that stopping the drug would resolve sexual problems in nearly all cases.

Will finasteride reverse complete hair loss?

No. If hair has been lost completely, finasteride will not make it regrow. It has not been shown to reverse a receding hairline, where hair has been lost at the front and side (temple region). It will not make hair regrow on an entirely bald area.

Can finasteride cause depression?

There are rare reports of finasteride being associated with significant depression. If depression or low mood occurs, treatment should be stopped immediately and a doctor consulted.

Are the improvements with finasteride long-lasting?

It can take up to 6 months for improvement, with maximum benefits usually at about 2 years. Once treatment is stopped improvement will usually be lost over a period of several months.

Is finasteride 1mg available through the NHS?

The treatment of hair loss is considered to be cosmetic treatment and is not normally funded through the NHS.

When is the best time of day to take finasteride?

Most men take their tablet in the evening. It can be taken at any time, although best to take at the same time each day to help with remembering.

I've just finished my first 168 tablets and I'm very happy with the effect it has on my hair. I've started taking one pill a day 3 weeks after my hair transplant for 6 months. Since then I've been taking 3 pills a week (mo,we,fr). My hair is looking great and I'm only getting compliments. I'm not aware of side effects and if there were any, the positive effects of having hair would outweigh them.

Not an anti finasteride post at all, but I do think my finasteride may be badly produced and I have been given an ineffective one by mistake. Been on it nearly 4 months and have not noticed any decrease in hair fall and have lost continual ground. Please help me out here.
